The purpose of this form is to obtain public
comments and proposals for changes/updates to the Specialty
Sleep Association’s Environmental & Safety Program as
published July XX, 2011. Comments/proposals as received by
SSA staff and no less than quarterly by SSA’s Green
Initiative and Board of Directors. Commenters will be
notified of committee review process and planned updates to
the SSA Program. Commenters may contact SSA staff to inquire

Each comment/proposal must
indicate: 1) Comment type, 2) Program level (i.e., 1, 2,
3, ALL, NEW Level) or license agreement to which
comment/proposal pertains, 3) Comment/Proposal and 4)
Reason for Change/Update (if a proposed revision is
being recommended).

Copyright Release
Statement. A signed copyright release is
required for all proposed changes/proposals submitted to
SSA. Only one copyright release is required for an
individual representing him/herself or a single entity.
A separate copyright release form is required for each
separate entity represented. General comments do not
require a signed copyright release.
