The purpose of this form is to obtain public comments and proposals for changes/updates to the Specialty Sleep Association’s Environmental & Safety Program as published July XX, 2011. Comments/proposals as received by SSA staff and no less than quarterly by SSA’s Green Initiative and Board of Directors. Commenters will be notified of committee review process and planned updates to the SSA Program. Commenters may contact SSA staff to inquire as to the timeline for a response to their request at (559) 868-4187 or info@sleepinformation.org.

 

PLEASE READ THESE INSTRUCTIONS CAREFULLY

  

  1. Each comment/proposal must indicate: 1) Comment type, 2) Program level (i.e., 1, 2, 3, ALL, NEW Level) or license agreement to which comment/proposal pertains, 3) Comment/Proposal and 4) Reason for Change/Update (if a proposed revision is being recommended).

  1. Comment/Proposal Type. The following codes must be used to indicate Comment Type:

  • SC = Substantive/Technical Comment

  • GC = General/Editorial Comment

  1. Copyright Release Statement. A signed copyright release is required for all proposed changes/proposals submitted to SSA. Only one copyright release is required for an individual representing him/herself or a single entity. A separate copyright release form is required for each separate entity represented. General comments do not require a signed copyright release.

  1. Complete, sign and return this Public Comment/Proposal Form by EMAIL or MAIL:

  • Submissions by EMAIL: Indicate SSA Public Comment/Proposal in the subject line of the e-mail message. Attach the signed copyright release form and public comment form file to the email message. Please ensure that all files are in MS-WORD or PDF. If appropriate, you may also attach other files with substantiating information. Email to info@sleepinformation.org.

  • Submissions by MAIL: Send your Public comments/proposals with signed copyright release to the:
